The attached Format questionnaire requests the information we plan to include on the site, but we can also include logos etc if you require.

You will be contacted each month to ensure that the site is always up to date so an e-mail address is important.  Please indicate if it is a private address so it will not show on the site but be kept for monthly update use only via a blind copy.
